Partnering Together
Throughout our journey together we will team you up with a designated Care Partner who is responsible for organising and monitoring your entire care. Your Care Partner is your direct contact person (we don't use call centres) and their role includes:
• Having ongoing conversations with you about your needs and goals; • Organising the care and support you require; • Involving you in managing your home care package to the extent that you prefer; • Working with you to update and co-produce care plans if your care needs change; • Providing you with clear monthly statements describing income and expenditure; • Organising for you to be formally reassessed, if you wish, to ensure that your care services continue to meet your needs.
Things that are important to your Care Partner:• What sorts of things might improve your day to day life?• What do you enjoy doing most?• What support do you need to stay safe?• Where and when do you want support?• How does this fit in with the help your carer provides you with?
Your Care Partner also considers any support you already have in place, such as carers, family members, friends, local community and other services, to work out the best way you can use your home care package funds.

Services
Your home care package funds can be used to purchase a wide range of services including: • Support services, such as help around the home, gardening, assistance with
shopping, visiting the doctor and attending social activities; • Personal care, such as help with showering, dressing and moving around the home; • Nutrition such as assistance with preparing meals, including special diets for health,
assistance with feeding; • Clinical care, such as nursing, allied health and physiotherapy for mobility and
strength.
